When patients ask me about improving sun damage, wrinkles, uneven pigmentation, or skin texture without surgery, I explain that there is no single “best” skin renewal treatment. CO2 laser resurfacing, chemical peels, microneedling, and IPL all work differently, and choosing the right approach starts with understanding your skin and what you want to improve.
As a board-certified plastic surgeon, I look beyond the treatment itself. I consider the type and degree of skin damage, skin quality, treatment goals, tolerance for downtime, and how nonsurgical treatments may fit into a patient’s long-term aesthetic plan. In some cases, a relatively gentle treatment is all that’s needed. In others, deeper resurfacing can produce a more meaningful improvement.
In this post, I’ll explain how 4 of the skin renewal treatments I commonly recommend work, what each does best, and how I determine which option may be appropriate for you.
CO2 Laser Resurfacing
CO2 lasers like the ULTRApulse® Alpha we use remove the outer layer of damaged skin to reveal the underlying smoother, softer, clearer, younger-looking skin below the surface. Removing the skin’s outer layer triggers the body’s natural healing response, which stimulates the production of new collagen.
It can improve sun damage, fine lines and wrinkles, uneven pigmentation, skin texture, and raised acne scars. Adding exosomes after your treatment supports the cellular signaling involved in skin repair and regeneration.
CO2 resurfacing can range from lighter treatments to more intensive resurfacing. I can adjust the laser’s energy and treatment depth based on each patient’s skin, concerns, goals, and tolerance for downtime. Because CO2 laser systems and treatment protocols vary significantly, deeper resurfacing requires careful patient selection and an experienced medical provider. I have more than a decade of experience performing CO2 laser resurfacing and customize each treatment rather than relying on a one-size-fits-all approach.
Chemical Peels
Chemical peels use carefully selected acids to exfoliate damaged skin cells and encourage smoother, brighter, more even-looking skin. The depth of treatment can range from gentle superficial peels to deeper resurfacing, allowing chemical peels to address different skin concerns and downtime preferences.
Lighter options include hydrating treatments such as lactic acid, a milk-derived alpha hydroxy acid (AHA), which gently exfoliates while supporting hydration. When performed as a series, lighter peels can gradually brighten and refresh the complexion with little or no visible peeling. Deeper chemical peels produce more significant resurfacing but typically require more recovery time.
Microneedling
Microneedling creates controlled microchannels in the skin that trigger the natural healing response and stimulate new collagen production. These channels can also enhance the topical application of exosomes following treatment.
Our approach uses traditional microneedling without radiofrequency (RF) energy or added heat. I prefer this method because it allows us to stimulate the skin without intentionally delivering thermal energy into the underlying tissues.
Because I perform one to two facelifts each week, I also consider how nonsurgical treatments may fit into a patient’s long-term aesthetic plan. I favor skin renewal treatments that can complement facelift surgery and preserve treatment options for patients who may consider facial surgery in the future.
IPL Photofacial
Intense pulsed light (IPL) is a non-ablative treatment that uses pulses of light to target unwanted pigment and visible blood vessels without removing the skin’s surface. It is especially effective for improving sun damage, brown spots, freckles, age spots, rosacea, and broken capillaries.
During treatment, pigment and visible capillaries absorb the light energy, which helps break down discoloration so the body can gradually clear it. Because IPL does not resurface the skin like a CO2 laser, it is a gentler option with minimal recovery time.
IPL is a good choice when uneven color, redness, or sun damage is the primary concern. With VISIA® Skin Analysis, we can assess surface concerns such as brown spots, redness, wrinkles, and enlarged pores and look deep into the layers of the skin to reveal the damage below that is not visible. For deeper wrinkles, significant textural changes, or acne scarring, I may recommend CO2 laser resurfacing or another treatment instead. In some cases, combining IPL with treatments such as microneedling allows us to address both skin tone and texture as part of a more comprehensive skin renewal plan.
